Please include the following information with any mailed check:
- Student Name
- Syracuse University ID Number (SUID)
- Return Address for organization or donor
- If the check is for the fall semester, spring semester, or academic year
Educational Resource Form
Any student who has been awarded an outside scholarship should submit the Educational Resource Form. The scholarship notification letter or email should be included when submitting this form.

If you are an outside scholarship provider or donor, please refer to the Syracuse University W-9 details.
